Edirol R-09 Digital Recorder
I love this little digital recorder. It has great sound and features one-touch recording (easy to use for even non-techies and technopeasants). After recording, you just plug it into your computer and drag the digital audio file (.wav or .MP3) to your desktop. From there you can edit it or put it on a website or turn it into a digital, downloadable audio product or file. Flip video
This is an amazingly small and easy to use video camera. It will record 60 minutes of HD video, fit in your pocket and then just pop out a USB connector to transfer videos to your computer or instantly upload them to YouTube. Great price. Great value. They are going out of production, so buy one before they are gone (and sorry if they are gone by the time you see this).
If you are a public speaker, you may want to have your own microphone system.

If you need an amplifier/mixer because you want to control your sound and the hospital (or future speaking venues) may not have a good one or one you can easily control, I'd recommend this one. It is more for local speaking engagements or ones you can drive to. Most amplifier systems are too big to bring on planes:

Great teleseminar service; you can even record the call for no cost and let people hear it later or download it for your own use as a product, bonus or get it transcribed and turn it into an e-book. They allow you to create web pages to market the telecalls, as well as let people participate in the calls over the web as an option instead of being on the phone. And they automatically record the calls and give you the option of downloading the recordings and/or letting people listen to replays of the calls.
